It may not be necessary for you to attend for an appointment. You can submit an e-consultation via the button below. You can submit details about your condition or your administration request and you will receive a response from the practice usually within 24 hours but you will be advised of the timeframe once you have submitted.
We will ask that you contact the surgery on the day you need to see a Doctor or Nurse Practitioner and we will endeavour to give you an appointment time to either be seen, or talk to a Doctor or Nurse Practitioner on the phone.
How does it work?
As of September 2017 the GP Partners at Church View Surgery have implemented a new appointment system.
Following careful consideration and advice from other surgeries this appointment system was designed to ensure our patients are dealt with safely and as quickly as possible.
All appointments are book on the day only. Appointments open up at 08.00 every morning and can be booked via telephone 01752 403206 or in person in reception.
A receptionist will ask you for a brief description of your symptoms to ensure they can signpost you to the correct medical professional. This could be the GP, Nurse Practitioner, Paramedic or Pharmacy. This also enables the GP and Urgent Care Team to prioritise those patients who need to be dealt with urgently.
All appointments are via telephone call in the first instance. You will be triaged over the phone then the GP or Urgent Care Team will book an appointment accordingly.
There are a small number of pre-bookable evening telephone calls however not all the GP’s offer this service.
Please note Nurse and Phlebotomy appointments can be booked in advance as normal.
Our telephone system has now been updated. We have more staff answering calls for the first hour. We are able to see the number of callers in a queue and the call waiting time. Please be aware the phones can be busier during peak times and we appreciate your patience as the team work through all the calls.
You may request an appointment with your usual doctor if you wish to maintain continuity of your care. If you know your consultation is likely to take longer than usual, perhaps because you have more than one problem, tell reception so that extra time can be allowed. Please make separate appointments for each member of the family.
When you arrive for an appointment make sure you check in at the Appointment Touch Screen by the main entrance or reception if you prefer before you sit down in the Waiting Room. Do let us know in good time if you are unable to keep an appointment. If you suspect an infectious condition, tell the Receptionist who will get a side room ready.
We can arrange for urgent cases to be seen at short notice, please make the urgency clear to the receptionist, but it will not always be possible to see the doctor of your choice.
If you wish to speak to the Receptionist in confidence there is a quiet area away from the main Waiting Room – please ask.
Appointments for the Nurse
We operate a booked appointment system for all Clinics. When booking your appointment please let Reception know what your appointment is for so that they can advise you when would be the best time for you to come and the length of appointment you require.
