Senior Partner Dr Rosie Heath Retirement

"It is with great sadness that the partners of Church View Surgery announce the retirement of Dr Rosie Heath, Senior Partner. Dr Heath has worked in the practice for 30 years, and we will all miss her energy, enthusiasm, clinical acumen, and her unfailing ability to look at every problem from an alternative perspective. She has decided to leave at the end of March.

If you would like to come and say farewell to Dr Heath, she will be in her office, taking tiger pictures off the walls, on 29 March 2018. She would be delighted to see patients and friends who may want to come and say goodbye - please just call in."

Church View Surgery: Registration Enquiries

From 5 December 2017, Church View Surgery will be capping our practice list and are therefore unable to take any new registrations. Due to our increasing list size and the current shortage of GPs, we feel that continuing to take more patients would jeopardise our ability to provide safe care to those already registered at the practice. You can find details of other practices at www.nhs.uk or ask a member of staff for assistance.

Church View Appointments: Patient Message

We are experiencing a period of high demand, and like other local practices, have been unable to recruit new GPs. During this difficult period we are doing everything we can to maintain a safe service. You can help us in the following ways:

  • Speak to your local pharmacist first for advice on minor ailments.
  • Please be as accurate as you can when describing your symptoms to the receptionists. It is their job to try to help direct you to the right person.
  • Please don’t use a phone appointment as a way of getting a repeat prescription – you can order repeat prescriptions online, or by dropping a request into the box.
  • Have your flu jab (we do still have nurse led flu slots available).
  • If you have a chronic disease, please come to your regular nurse led reviews, as we will try to help you to stay well.
  • Be aware that you can self-refer to the physiotherapy service at Derriford – see the Trust website at www.plymouthhospitals.nhs.uk
  • There are a number of online resources, such as Patient UK and NHS Choices, that have information about common conditions. They give advice on when to consult your GP.
  • You can self-refer to the Psychological Therapies service at Plymouth Options, by ringing Plymouth 435419.

We wanted you to know that this is not the service we want to provide, but we are doing our best in the circumstances in which we find ourselves. Your help and patience are very much appreciated.


 Change of Clinical System

Information Sharing

One of the advantages of our new clinical system is that it allows us to better share information with other teams involved in your care, e.g. District Nurses, Mental Health team. We will assume that you don’t mind your information being shared, but if you do not want anyone outside the surgery to see your record, please notify one of our team, and we can record your preferences accordingly. If you have any questions about this, please contact Emma Rayment, Practice Manager and Caldicott Guardian.
