New Appointment System

Welcome to our Practice Website

Welcome to The Church View Surgery, providing primary care for Plymstock and surrounding areas. We hope you take your time to browse our website where you will find lots of useful information about the surgery and the services we offer.

Change of Clinical System

We are currently in the process of updating our clinical system with a go live date of Thursday 8th June.

Please be aware that any patients registered for online services will have to re -register on the new system after the go live date. Our receptionists will be happy to help with this. Please remember to bring ID with you for verification.

Apologies for any inconvenience this may cause.

Information Sharing

One of the advantages of our new clinical system is that it allows us to better share information with other teams involved in your care, e.g. District Nurses, Mental Health team. We will assume that you don’t mind your information being shared, but if you do not want anyone outside the surgery to see your record, please notify one of our team, and we can record your preferences accordingly. If you have any questions about this, please contact Emma Rayment, Practice Manager and Caldicott Guardian.

About Our Practice

The practice is bounded to the North by the A38, to the South by the coast, to the West by the Laira Estuary and Plymouth Sound and to the East by the village of Yealmpton. The Practice covers a large population and currently services over 12,000 patients.

We have been approved to provide General Practice Vocational and Medical Student Training. This enables qualified Doctors to work with us for up to 1 year under supervision prior to joining a Practice. Students from the Peninsula Medical School join us several times a year as part of their training. You may be asked if a Student may interview or examine you as part of their assessment by the Practice. We hope you would grant them this opportunity but respect your wishes should you decline.

On The Day Appointments

We are introducing a new way for you to access on the day appointments and advice from Monday 31st October 2016. We know that our patients have problems getting on the day appointments and advice here at Church View. The demand for GP appointments in England is growing at a significant rate at the same time as there is a shortage of GPs and the funding to employ them.

Please download this leaflet for further information: On The Day Appointments

Dr Mike Price is now working at Church View as a locum doctor. This means that patients registered with Dr Price will be allocated to Dr Paul Graham who will be joining the practice in early January 2017 on a permanent basis. Locum doctors do not hold a list of registered patients.

Dr Helen Kimble will be working a reduced number of sessions at the practice from 1st November 2016 and her patients will be allocated to another GP in the practice.

Diabeteshelp@Plymouth Project

Plymouth Hospitals NHS Trust, Plymouth University and Sentinel Healthcare are working together to develop online email support to help:

  1. those who are newly diagnosed with type 2 diabetes to access information or support through using the internet; and
  2. those who can use the internet a little (i.e. can use email) but need more help in using it for diabetes.

The support is given by trained volunteers who themselves have type 2 diabetes.
